Series ini bercerita tentang kisah para mahasiswa yang maggang dirumah sakit untuk menjadi dokter bedah yang baik. Malam itu, Meredith Grey tidur bersama Derek dan terbangun dengan buru – buru ketika akan telat dihari pertama maggangnya.
Betapa terkejutnya Meredith ketika dia mengetahui bahwa Derek adalah calon pembimbing dia dirumah sakit itu. Berada diantara menyembunyikan perasaan atau perjalanan menjadi dokter, Meredith harus menerima kenyataan bahwa Derek sebenarnya sudah memiliki istri. Dan semua bertambah parah ketika istri Derek kembali bekerja dirumah sakit yang sama.
Other side, ada juga Cristina yang hamil ditengah perjalanannya menjadi dokter. Hamil anak Burker, saingan berat Derek untuk menjadi kepala rumah sakit disana.
Well, Cristina yang tidak memberitahu Burke harus menerima fakta bahwa dia mengalami keguguran.

Malley is getting syp and knowing that Alex make him having that. Their friendship then broke because Malley look really mad.
Grey starting to trust Derek and telling him about her mother situasion.
In the end of season, Addison appears and introduce her self as a Derek wife. What gonna happen between Derek and Grey after Addison saying that Grey screw her husband ?
Richard also know about Grey relation with Derek.
